Several years ago a thread came up in a bird group I was on, about how to notify first responders of pets we may have at home, so I borrowed an idea (from an attorney who represented pets) & redesigned it to fit the group's needs & thought it might be of interest here.
The thread was started by a member who has since passed, but then, that was the whole point of the thread.
Many of us live alone and/or have friends or relatives who may not be close by and if we are incapacitated or are unable to communicate, the idea was that this care notice could do it for us.
Today, most of us have some sort of text program on our computers that will let us fill out both sides of this notice, then reduce it to wallet size, take it to Office Depot, Staples or other printing services store where we can have it laminated if we want.....some of us may even have all the capabilities at home or at work and can do it all ourselves.....
The images are in .JPG format, so they can be blown up to work with/fillout.....
